There are two forms shown online for Nursing Sister Copps.  The first one from 1917 is filled in above.  After this she served at Queen's Military Hospital.  The second form is an "Officers' Declaration Paper" dated April 16th, 1918 and signed at Kingston, Ontario.  This form is stamped "No. 10 Draft, Nur. Sisters".
