The November 19th, 1917 attestation is not signed and a location is not given.  The medical examination that accompanies it was completed in Calgary, Alberta.

There is a second form for C. L. Bishop - an "Officers' Declaration Paper".  She completed this form on April 11th, 1918 in Calgary, Alberta.  Here she stated that she had four months previous military experience as a Nurse in the C.A.M.C.
