There are two attestations on file. The first 1915 attestation was approved by ? Macdonald, Matron-in-chief, for D.M.S., Canadian Contingents.

The second attestation is in the form of an Officers' Declaration Paper and is marked as a "Duplicate Copy" and includes a May 12, 1917 medical examination. On this form her present and next-of-kin addresses are R.R.4, New Glasgow, Nova Scotia, and her father's name is given as W. H. D. MacKay.
